 Reservation Request Form

Please use the Reservation Request Form to request a rental date if you are booking more than 6 months in advance. If it is your first time renting the theatre, please set up a time to meet with our Theatre Director prior to submitting your request. This request must be received with a non-refundable rental deposit AND approved by the Theatre Director to officially reserve a date.


Reservation Agreement

Please use the Reservation Agreement Form to request your rental date if you are booking less than 6 months in advance or to complete the details for your event after previously submitting a reservation request. You will use the form to specify dates and times you need access to the theatre, hours for technicians, and amenities needed. If it is your first time renting the theatre, please set up a time to meet with our Theatre Director prior to submitting your request. This form must be received with a non-refundable rental deposit AND approved by the Theatre Director to officially reserve a date.


Box Office Agreement

Please use the Box Office Agreement Form if you plan to use the Paramount Theatre Box Office Service for selling tickets to your event (this is highly suggested for all ticketed events). Effective January 1, 2014, N.C. Gen. Stat. § 105-164.4(a)(10) imposes a privilege tax on the retailer’s net taxable sales or gross receipts to admission charges to an entertainment activity. All advertised ticket prices for events sold through the Paramount Theatre Box Office Service include 6.75% Admissions Tax when applicable. If you do not use the Paramount Theatre Box Office Service, you are responsible for collecting and reporting these taxes.


Box Office Waiver

Please use the Box Office Waiver form if you DO NOT plan to use the Paramount Theatre Box Office Service for selling tickets to your event. This form is required in addition to the Reservation Request and/or Reservation Agreement if you do not use the Paramount Box Office Service. Effective January 1, 2014, N.C. Gen. Paramount Theatre Supplement Guide

This document contains important information such as details on Fire Watch, Alcoholic Beverages, Film Showings, Poster/Graphics sizes for display in the Paramount lobby, Basic Advertising Contacts, and specifications of the theatre such as stage size, seating charts, and lights/sound equipment.


E-Verify Affidavit Form and IRS Form W-9

These forms are required for first time users of the Paramount Theatre Box Office Service or when changing the individual or organization that is receiving the payment for ticket sales. Both forms must be completed, signed, and the Affidavit form must be notarized.
